Information for Funeral Directors
Westchester County Medical Examiner Mortuary Services
In order for the Westchester County Medical Examiner’s Office to release remains, Funeral Directors are required to present the following:
- A completed affidavit form for removal.
- A working copy of the death certificate.
- The Medical Examiner Case Number.
- To obtain a case number, please refer to the authorized family representative or next of kin, the Electronic Death Registration System (DAVE) or by calling our main number 914-231-1600.
- A valid New York State funeral director's license.
- A valid driver's license or other government-issued ID.
Mortuary release hours:
Monday through Sunday 8:00AM to 3:45PM.

To obtain a non-communicable disease form:
Please contact our office to notify us of the need for a non-communicable disease form prior to arriving. A completed form, along with notarization (based on notary availability), will be provided during body release.
